- 設計：這是2019–2025年在中國6個城市、12家伴侶動物醫院進行的多中心分子流行病學監測研究，結合糞便與環境採樣、抗菌藥物感受性試驗、全基因體定序、cgSNP親緣分析與質體比較。研究主軸不是估計臨床預後的HR／OR，而是追蹤blaNDM-5陽性Escherichia coli在獸醫院、動物與環境間的群聚傳播。
- 樣本：研究收集341件犬貓糞便樣本（185犬、156貓）與Hospital A的392件環境拭子；糞便樣本有36件（10.6%）分離出38株blaNDM-5陽性E. coli，環境樣本有44件（11.2%）分離出48株blaNDM陽性菌，其中3株為blaNDM-5陽性E. coli。總計41株blaNDM-5陽性E. coli接受全基因體分析，另納入GenBank 129株寵物來源blaNDM陽性E. coli與85株寵物來源ST410作全球親緣比較。
- 關鍵數字：41株中36株（87.8%）為ST410；糞便來源38株中35株（92.1%）為ST410，且本研究36株ST410分屬B4/H24RxC（n=10）與B5/H24RxC（n=26），同院株間僅0–7 cgSNP、Hospital A環境株與4個月前動物株僅3–7 cgSNP，支持院內株系傳播而非偶發檢出。抗藥性訊號很強：41株全數對imipenem、ampicillin、cefoxitin、ceftazidime、cefotaxime抗藥，gentamicin與ciprofloxacin抗藥率分別>97.6%與95.1%，但tigecycline全數仍具感受性；全球資料中ST410占129株的38.0%（n=49），寵物株與人源株可接近到≤9或16–19 cgSNP，效應量在基因體流行病學上相當有說服力。
- Limitation：研究為監測型與基因體關聯分析，沒有配對飼主、醫護人員或時間序列接觸資料，因此≤25、≤20或≤9 cgSNP只能支持跨物種傳播的高度相容性，不能證明傳播方向。環境採樣集中在Hospital A且發生於2025年2月，其他醫院缺乏同等環境監測；選擇性培養使用0.5 μg/mL meropenem且每盤僅挑1–2個菌落，也可能低估菌種多樣性與共存質體。
- Bottom line：伴侶動物醫院已可成為NDM-5-producing E. coli ST410的院內儲存庫與放大器；臨床上應把獸醫院感染管制、環境清消、CRE篩檢與One Health AMR監測視為同一條防線。
