Andes virus-associated cardiopulmonary syndrome: diagnostic approaches, clinical management, and evidence gaps-a scoping review

- 設計:這是scoping review,搜尋PubMed/MEDLINE、Embase、Scopus、Cochrane Library與ClinicalTrials.gov共5個資料庫,涵蓋1993-01-01至2026-05-13的ANDV-HCPS臨床文獻。它把診斷、支持性治療、ECMO、antiviral與immune plasma等問題整理成1個臨床管理框架。
- 樣本:共納入40篇研究,多數來自智利與阿根廷,研究型態以retrospective cohorts、case series與case reports為主。可用證據幾乎沒有randomized trial,追蹤資料也以急性住院期為主,長期post-discharge morbidity資料明顯不足。
- 關鍵數字:診斷上serology是常規主力,RT-PCR多用於診斷困難個案,且0個validated ANDV-specific point-of-care test;治療上仍是mechanical ventilation、vasopressors與ECMO等支持性照護。ECMO在specialist centres與較佳存活相關,但效應量來自觀察性資料,代表訊號重要卻不夠穩健。
- Limitation:40篇研究多為回溯與個案報告,selection bias、referral bias與survivor bias都可能放大ECMO等高階照護的表面效益。診斷標準、嚴重度分層與升級照護門檻未在各研究間一致化,讓跨研究比較至少有3層異質性。
- Bottom line:ANDV-HCPS應被視為可跨境擴散的高致死One Health急症,臨床重點是早期serology/RT-PCR路徑、明確升級照護標準,以及把重症個案快速轉到具ECMO經驗的中心。

One Health genomics reveals niche-specific lineage replacement in Salmonella Enteritidis

- 設計:這是全國尺度的One Health genomics研究,整合中國人類、動物、食品與環境4個界面的S. Enteritidis isolates。分析重點是20年間lineage dynamics、mobile genetic elements、stress tolerance、metabolic plasticity與human-centric dissemination。
- 樣本:摘要未揭露總isolate數,但明確指出這是中國目前最大S. Enteritidis genomic資料集,涵蓋20年與至少4個來源界面。比較組不是傳統2組臨床cohort,而是Global-b1、Global-b2與Global-c共3個主要lineages的時序與生態位比較。
- 關鍵數字:核心發現是MDR率在20年間增加3倍,且Global-c透過resistance-driven lineage replacement取得比例優勢。這個效應不是小幅漂移,而是AMR、mobile genetic elements與環境適應共同推動的lineage replacement,對公共衛生監測的訊號相當大。
- Limitation:基因體監測受sampling frame影響很大,4個界面的採樣密度若不均,可能把監測偏差誤認為lineage replacement。SNP-distance mapping可提示human-centric dissemination,但不能單獨證明傳播方向,也缺少與臨床嚴重度或治療失敗直接連結的數字。
- Bottom line:對AMR監測來說,S. Enteritidis不能只看serovar或單株AST,應把lineage、mobile genetic elements與跨界面傳播納入One Health surveillance。
