Risk factors and outcomes of Pneumocystis jirovecii pneumonia after allogeneic hematopoietic stem cell transplantation for hematological malignancy: A 10-year cohort study

- 關鍵數字:PJP盛行率為1.0%(88/8596),但預後差距很大:5年OS 58.7% vs 84.3%、DFS 57.3% vs 82.0%、NRM 33.0% vs 8.5%,且29.2%發生在移植後1年以上,顯示效應量臨床上很大,不只是統計差異。
- Limitation:retrospective設計容易有prophylaxis adherence、corticosteroid累積劑量、aGVHD/cGVHD嚴重度與診斷門檻的confounding;16/88例因資料不完整或失訪排除,也可能造成selection bias。
- Bottom line:allo-HSCT後PJP雖只有約1%,但死亡與NRM代價高;年齡>45歲、高峰corticosteroid劑量、低CD4與cGVHD病人應考慮更長或更個別化的prophylaxis。
