Improved Tolerability of De Novo Letermovir Over Valganciclovir for Cytomegalovirus Prophylaxis in Seropositive Liver Transplant Recipients¶

- 設計:這是2021/6/1至2024/9/30成人CMV R+ liver transplant的回溯性世代研究,比較de novo letermovir與valganciclovir預防。主要問題不是證明letermovir優於標準治療,而是看1年內耐受性、安全性與臨床可行性。
- 樣本:共85名受試者,valganciclovir組60人、letermovir組25人;追蹤至移植後1年,並比較3個月與6個月mycophenolate總日劑量。兩組人數不大且比例約2.4:1,特別是letermovir組只有25人,估計值會受單中心用藥選擇影響。
- 關鍵數字:letermovir完成預防比例較高,92.0% vs 68.3%(p=0.02),valganciclovir組轉換策略中位時間為移植後58天,這是臨床上有感的差距。letermovir組leukopenia較少(52.0% vs 81.7%, p=0.005)、neutropenia較少(24.0% vs 50.0%, p=0.03)、GCSF需求較少(4.0% vs 25.0%, p=0.024),效應量不小且方向一致;1年CMV viremia與acute rejection無顯著差異,但樣本數不足以排除中等幅度差異。
- Limitation:主要限制是回溯性、非隨機分派與單中心處方偏差,letermovir被選用的25人可能本來就有不同的骨髓抑制風險或免疫抑制策略。CMV viremia、acute rejection與mycophenolate劑量之間也有confounding by indication,且研究未能以85人可靠評估罕見但關鍵的ganciclovir-resistant CMV或長期graft outcome。
- Bottom line:對CMV R+ liver transplant,de novo letermovir在1年資料中看起來可減少骨髓抑制並提高預防完成率,但目前更像是高風險耐受性策略,而非已取代valganciclovir的定論。

- 設計:這是深圳單一三級醫院2021/12至2025/12的4年回溯性研究,納入疑似lower respiratory tract infections且同時做BALF mNGS與conventional culture者。設計重點放在檢出率、polymicrobial identification、年齡與共病分層病原分布,以及mNGS-only結果對診斷與抗微生物治療的臨床影響。
- 樣本:共841名住院病人,所有人都有BALF paired testing,因此比較基礎比許多只看mNGS陽性個案的研究更完整。追蹤時間以臨床處置影響為主,摘要未提供固定30天或90天outcome,也未列出各組免疫低下者人數。
- 關鍵數字:mNGS的整體與polymicrobial detection均顯著高於culture,且超過50%的病原只由mNGS檢出,其中又有超過50%影響診斷或抗微生物治療,代表效益不只是實驗室陽性率增加。特別值得注意的是Mycobacterium tuberculosis complex、nontuberculous mycobacteria、Cryptococcus neoformans與Pneumocystis jirovecii即使read counts低仍有臨床意義,這對免疫低下與難治肺炎判讀很實用。
- Limitation:這類研究最大的限制是retrospective clinical impact判定容易有reviewer與documentation bias,且mNGS陽性不等於感染,colonization、污染與死菌DNA都會放大檢出率。以culture作比較也不等於完整reference standard,若沒有獨立adjudication、抗菌藥暴露分層與一致的BALF採樣時機,841人的規模仍不能直接換算成診斷準確度或存活效益。
- Bottom line:BALF mNGS在疑似LRTI特別是混合感染、抗生素失敗、TB/NTM、Cryptococcus與PJP情境有實際加值,但報告必須和宿主風險、影像、定量訊號與傳統微生物結果一起判讀。
Free-ranging hedgehogs as sentinels of environmental extended-spectrum β-lactamase-producing Enterobacterales¶

- 設計:這是以色列全國性free-ranging hedgehogs糞便帶菌監測,研究ESBL-producing Enterobacterales與carbapenem-resistant Enterobacterales在都市周邊野生動物的分布。設計把野生動物放在human-animal-environment介面,適合作為One Health AMR surveillance的補充框架。
- 樣本:2022/12至2023/9共246隻hedgehogs,來源為全國救援後送至唯一wildlife hospital,入院時採檢並對所有分離株做antimicrobial susceptibility testing。追蹤是橫斷面入院採樣,沒有個體長期追蹤或環境、人類、寵物同步採樣。
- 關鍵數字:ESBL-PE帶菌率高達71%,CRE為2%,以E. coli為主,其次為Citrobacter、Klebsiella與Enterobacter spp.;quinolone resistance達83%、trimethoprim-sulfamethoxazole resistance達46%,這不是零星污染,而是高壓力AMR背景的生態訊號。帶菌率在假期與戶外活動增加月份較高,雖然只是時間相關,但提示人類活動可能改變野生動物暴露。
- Limitation:最大限制是selection bias,246隻都是被救援且送醫的hedgehogs,不能直接代表所有野外族群。研究摘要未顯示WGS、plasmid typing或與人類臨床株的基因體比對,因此71%高盛行率能證明哨兵價值,但尚不能證明傳播方向或來源。
- Bottom line:都市共棲野生動物可成為AMR環境壓力的早期哨兵;若下一步加入人類、污水、寵物與基因體監測,hedgehog surveillance可能補上傳統臨床AMR監測看不到的一塊。
